Welcome to HCA Florida Lake City Hospital! We are very excited that you have decided to join our team. You should have received an email from your recruiter with information about the pre-employment process. Please do not hesitate to stay in touch with HR on any questions you have during the course of this process. There are 3 steps to completing the pre-employment process:

Step 1: Application Update and Additional Questions

As part of our New Hire Process, we will need you to provide additional information about yourself. You should have a link in the email you received to access your information. Please contact your recruiter if you have not received this information.

Step 2: Background Authorization Release

You should have received the link via email from your recruiter to complete the authorization within 24 hours. The link enables you to complete the release authorization electronically to expedite the process.

Step 3: Pre-employment Health Assessment

You should have been scheduled for a pre-employment health assessment by your recruiter. Should you need to reschedule, contact your recruiter or HR directly for assistance. Any rescheduling will delay your start date. Be sure to bring the following items with you to your appointment:

Human Resources cannot review or accept this information. This information should be provided to Employee Health only. Do not provide this information to Human Resources.

  • Documentation of PPD in the last 12 months
  • Immunization Records or Titers
  • Review and bring completed Pre-Employment Screening Paperwork. Company Care is located just 2 blocks east of the hospital.

Important Information:The documents and information requested are for the purpose of obtaining a complete health profile and to comply with the Americans with Disabilities Act. Further, the information will allow your employer to evaluate and provide reasonable accommodation for any qualifying disability you may have. This information will be kept confidential in a separate medical file, apart from your personnel file. Any employee who falsely represents his/her condition in writing at the time of entering into the employment relationship with the employer may be denied Worker’s Compensation benefits; in addition, any false representation at this time may subject the employee to termination.

The Genetic Information Nondiscrimination Act of 2008 (GINA) prohibits employers and other entities covered by GINA Title II from requesting or requiring genetic information of employees or their family members. In order to comply with this law, we are asking that you not provide any genetic information when responding to this request for medical information. ’Genetic information,’ as defined by GINA, includes an individual’s family medical history, the results of an individual’s or family member’s genetic tests, the fact that an individual or an individual’s family member sought or received genetic services, and genetic information of a fetus carried by an individual or an individual’s family member or an embryo lawfully held by an individual or family member receiving assistive reproductive services.

Step 4: New Hire HR Paperwork

You have also been scheduled for a New Hire Paperwork appointment in Human Resources. Any rescheduling may result in a delay your start date. Be sure to bring the following items with you to your appointment:

  • Voided check for direct deposit
  • License plate # for your parking decal
  • Original Certification Cards (BLS, ACLS, etc.)
  • Social Security Card (for payroll purposes)
  • Form I-9 Original Identification Documents (reference acceptable forms of identification in the listing included in the New Hire Paperwork)
